Protamine sulfate antidote to overdosage with heparin if used in excess protamine has an anticoagulant effect allergic reactions increased in persons at risk. Hepatic Dose : No dosage adjustments are recommended. Protamine sulphate is a basic protein derived from fish sperm that binds to heparin to form a stable salt. Renal Dose : Dose in Renal Impairment GFR (mL/min)ĭose in Patients undergoing Renal Replacement Therapies Protamine sulfate neutralizes roughly 100 units of heparin in one milligram. Interaction :Ĭephalosporins and Penicillins: Protamine has been shown to be incompatible with certain antibiotics, including several of the cephalosporins and penicillins. Protamine sulfate is a basic protein produced from fish sperm that forms a stable salt with heparin. Methods: We evaluated the effects of increasing protamine concentrations (0 to 24 microg/mL) on prothrombin time and diluted Russell's viper venom time measurements on thrombin generation in platelet-poor and platelet-rich plasma after activation by tissue factor or actin, and on. Anaphylaxis, systemic hypertension, nausea, vomiting, lassitude. Background: Protamine sulfate is the antidote for heparin, but in excess it exerts weak anticoagulation. ![]() Sudden fall in blood pressure, bradycardia, pulmonary hypertension, dyspnea, or transitory flushing and a feeling of warmth. ![]() Administer within 15 minutes-if the delay is longer, less protamine is required, as heparin is rapidly excreted (after 30 minutes give approximately half dose). Protamine Sulfate Injection, USP is a sterile, isotonic solution of protamine sulfate. If 4-8 hours after dose: give 0.5 mg for each 1 mg of enoxaparin or 100 units of dalteparin and tinzaparin. 1 mg for each 100 units low molecular weight heparin (LMWH) or 80 units for bovine heparin given at last dose. Repeat one-half dose of protamine in 4 hours. ![]()
